    To begin enrollment, please reach out to one of our Social Workers at 916-648-3999. We’re here to guide you every step of the way.
  • Center Tour
    Schedule a visit to AltaMedix ADHC and experience a typical day at our center. This tour allows you to see firsthand how our CBAS services can support your health and social needs.
  • Physician Referral
    We’ll request that your physician complete a History and Physical Form. This provides us with up-to-date information about your medical history, current medications, dietary needs, TB clearance, and any necessary therapy authorizations.
  • Managed Care Assessment
    A nurse from your Managed Care Plan will conduct a face-to-face assessment to determine your eligibility and medical necessity for ADHC/CBAS services. This can take place at the center or in your home.
  • Three-Day Initial Evaluation
    Our multidisciplinary team—including a Registered Nurse, Medical Social Worker, Physical Therapist, Occupational Therapist, and Activity Coordinator—will assess your health and social needs over three days. A Social Worker will also conduct a home visit to evaluate your environment. Additional specialists, such as a Registered Dietitian, Mental Health Professional, or Speech-Language Pathologist, may be included if needed.
  • Plan of Care Development
    Based on the assessment, our team will determine if the program can meet your needs. If accepted, we will create a personalized six-month care plan and officially enroll you into the program.
  • Ongoing Reassessment and Coordination
    Your care plan will be reviewed and updated at least every six months. Any changes in your condition will prompt a reassessment, and our nursing team will coordinate with your physician to update treatment orders and services as needed.
